What Is A Medicare Chronic Disease Management Plan?

We understand that every workplace injury is different. Some workers benefit from clinic based rehaA Chronic Disease Management (CDM) Plan is a referral from your GP that may provide Medicare rebates for allied health services, including Exercise Physiology.

You may be eligible if you have a medical condition that has been present, or is expected to be present, for six months or longer.

Your GP can determine whether you are eligible and provide the necessary referral.
